Please print, fill out, and mail the completed packet and $135 check, made out to FFSAZ, to P.O. Box 1458, Gilbert, AZ 85299. 

We now have the ability to accept payment through Zelle. You can send Zelle payment to ffsazinfo@gmail.com and mail only your registration packet. Teaching assignments are made on a first come and class need basis.
